Community Care Resources of FL (CCR) is committed to providing high-quality primary care and behavioral health services to everyone, regardless of their ability to pay.
We offer a Sliding Fee Discount Program for patients who are uninsured or underinsured. This program allows us to reduce or “slide” the fees for our services based on your household size and annual income.

• No Denial of Care: No one will be denied access to services because of an inability to pay.
• How to Apply: Discounts are available to all patients who meet the eligibility requirements based on the 2026 Federal Poverty Guidelines. To apply, please ask our front desk staff for a Sliding Fee Application or download it here [Link to PDF].
• Nominal Fee: Patients at or below 100% of the Federal Poverty Level will be charged a small nominal fee of $10.00 per visit. This fee may be waived if a patient is unable to pay.
• Insurance: We accept Medicaid, Medicare, and most private insurance plans. Our Sliding Fee Discount can also be applied to any remaining patient balances for those who qualify.
