Community Care Resources of Florida (CCR) is a patient-centered healthcare provider committed to delivering accessible, high-quality primary care and preventive health services to the communities we serve. We offer comprehensive medical care in a respectful, compassionate, and confidential environment where every patient is treated with dignity.
Our services include primary care visits, health screenings, testing and treatment for sexually transmitted infections (STIs), and treatment for non-sexually transmitted infections, along with ongoing care that supports overall wellness. While we do not offer abortion services, we are dedicated to providing education, early detection, and appropriate medical treatment to help patients make informed decisions about their health.
Primary Care
Our primary care services provide comprehensive, patient-centered care focused on prevention, early diagnosis, and ongoing health management. We offer routine checkups, chronic condition management, screenings, and personalized treatment plans in a compassionate and respectful environment. Our goal is to support your long-term wellness by being your trusted partner in everyday healthcare.
Medical Care Services
Comprises physical examinations and screenings for conditions such as blood pressure, cholesterol, diabetes, cancer, asthma, HIV, COPD, coronary heart disease, and Hepatitis C. We also offer annual and routine immunizations and vaccines.

Wellness Services
CCR provides wellness services with compassionate care. These services include but are not limited: primary care, women’s and men’s health.
Herpes Simplex Virus I/II (HSV I/HSV II)
Screening for HSV 1/HSV 2. If you are experiencing symptoms we do offer treatment.
Symptoms may include:
(1) If you are experiencing blisters around the mouth, genitals, or rectum, testing can be an option.
(2) These blisters might give a sensation of burning, tingling, or numbing around the affected area.
(3) Some people experience flu-like symptoms or at times, there are no symptoms at all.
If you believe you have been exposed, please let us know. Most people who have been infected, don’t even know they have the virus.

HIV (Human Immunodeficiency Virus)
Confidential Rapid HIV Antibody Test and HIV Early Detection Test.
HPV (Human Papillomavirus)
CCR does not test for or treat HPV. Please make an appointment with your primary care physician.
Syphilis
CCR offers Rapid Syphilis Testing.
- Readily accessible rapid syphilis testing and conventional, lab-based syphilis testing for individuals at increased risk for syphilis
- Syphilis testing at low or no cost to individuals who would not otherwise be able to afford testing
- Client-centered counseling designed to reduce client risk of acquiring or transmitting syphilis
- Appropriate follow-up and care to treat and cure identified syphilis cases
Who should get tested for Syphilis?
- Men who have sex with men (MSM)
- People living with HIV
- People who are pregnant
- People who have been diagnosed with a gonorrhea or chlamydia infection
- Partners of people who have tested positive for syphilis People who are sexually active and live in areas with high syphilis rates
- People who are taking PrEP for HIV prevention
- People in correctional facilities
Hepatitis C
CCR offers Individual Hepatitis screening and testing. Treatment is available.
We perform the Rapid Test for Hepatitis C. Hep C which is spread by sharing needles & syringes. Hep-C can also be spread through sexual contact with an infected person or receiving tattoos/piercing from unlicensed individuals. The Hep C test is done via Rapid Test kit in office. Takes 15 mins for results.
We also check for Hep B which is spread via sexual contact, blood, blood products, IV needles, contact with semen and bodily fluids every three months for the patients on the prep program.
We do not test for Hep A which is a food-borne illness. Please see your primary care provider for this test and possible treatment.
Mental Health & Substance Abuse
CCR offers a range of on-site services encompassing Mental Health and Substance Abuse.
